AmeriCorps VISTA at Sunrise Community Health Center

Sunrise is requesting the assignment of 1 VISTA who is interested in improving the health & wellness of uninsured & under insured individuals. The assignment to Sunrise will be a 1 year endeavor & will focus on building programs that will enhance: patient access, communication, & engagement, quality of care, educate patients about Sunrise services, & research resources that will benefit our patients. SCHC began in 1973 as a migrant health clinic. Today, our family medicine, pediatric, prenatal, dental, & school-based health centers are health care homes for more than 34,000 people in northeastern Colorado. We are a private, non-profit 501c3 supported in part by community health center funding through the Bureau of Primary Health Care. Sunrise offers comprehensive, quality, & affordable medical, dental, & behavioral health services including on-site laboratory, radiology, pharmacy, & patient education services. All services are offered to people in need regardless of their ability to pay. Sunrise is a recognized leader in integrated care, advanced Health Information Technology (HIT), safety net collaborations, & professional health education. In addition, Sunrise co-founded the North Colorado Health Alliance, a community collaboration focusing on the low-income, under & uninsured people within our service area. Partners include community health, public health, hospital, behavioral health, specialists, local foundations, education, county commissioners, managed care organization, & county social services. Collaborations focus on integrated service expansion; shared HIT infrastructure; workforce development; community health improvement; regional accountable care activities; & system accountability & efficiencies. Member Duties : Sunrise will provide a professional environment where a VISTA will be able to hone their skills in project management, communication, creativity, developing community relationships, and teamwork. We will support the VISTA by providing direct communication, set clear and attainable deadlines and they will have direct supervision from the Quality Improvement Director. The projects listed in the VAD will allow Sunrise to expand and grow our programs through outreach, education, program development, and building partnerships with community partners. These objectives will be sustainable at Sunrise once a foundation has been laid by rolling these duties into the job descriptions of current employees and other volunteers. By having the assistance of a VITSA Sunrise will be able to outreach and provide services and education to more patients. It is the hope of Sunrise that these endeavors will ultimately improve the wellness and health outcomes of the population that Sunrise serves.
