Supports Coordinator

Primary Purpose:

 

Serves as supports coordinator for children and families enrolled in program serving developmentally disabled children age 0-17. The supports coordinator is identified as the chairperson of the multi-disciplinary team and the developer of the Person-Centered Plan. The coordinator is responsible for assessment, referrals, linking and coordination of all services, as well as for monitoring and supervising prescribed services. The supports coordinator is the responsible party for the clinical record.

 

Education and Experience Required:

 

1.       Bachelor’s degree in psychology, social work or related human service field required. Master’s preferred.

2.       Must have the ability to receive QIDP status through Wayne County and agency trainings.

3.       Experience in working with and assessing children according to their individual needs and developmental level.

4.       A minimum of three (3) years’ experience working with developmentally delayed, mentally ill, and/or disabled children and their families required.

5.       Experience working in the home and/or community setting preferred.

 

 

Knowledge, Skills and Abilities Required:

 

1.       A range of knowledge in mental health topics, techniques, theory and service delivery with children and families 0-17 required as well as service systems and levels of care.

2.       Comprehensive assessment skills.

3.       Direct clinical experience performing group and/or individual psychotherapy with clients along with the empathy and compassion necessary to facilitate positive growth.

4.       Demonstrated knowledge of child, adolescent and/or adult conscious and unconscious psychodynamics, developmental stages and signs of conflict or distress.

5.       Competency, or willingness to learn, co-occurring treatment of mental health, substance abuse and/or developmental disabilities.

6.       Development and implementation of Individualized Plan of Service utilizing Person Centered Planning.

7.       Knowledge of theory and practice of individual, group and family systems treatment.

8.       Competence in the delivery of crisis intervention services, and in brief and time-limited therapy.

9.       Competency, or willingness to learn, a wide range of treatment modalities that are individualized and person centered.

10.     Ability to develop treatment plans and coordinate needed services to fulfill plans.

11.     Demonstrated ability to set and meet program outcomes and objectives.

12.     Ability to communicate effectively both orally and written.

13.     Ability to plan ahead and organize time effectively.

14.     Ability to assist families in crisis.

15.     Ability to present self in a professional manner to staff, community, other social service agencies and referral sources.

16.     Ability to problem solve and make decisions, especially during crisis situations.

17.     Must have a working knowledge of all SFS programs within the first 90 days of employment.

18.     Ability to manage multiple priorities, function independently and demonstrate good follow through.

19.     Ability to work positively, cooperatively and productively as a member of a team with colleagues, supervisors and agency staff.

20.     Computer literacy, including PowerPoint and excel.

21.     Cultural sensitivity/competence working with multicultural client populations

22.     Capacity for developing autonomy and leadership in supervisees.

23.     Ability to maintain of ethical and professional standards.

24.     Must respect the confidentiality of all program participants.

25.     Must possess a current State of Michigan driver’s license and automobile insurance that is insurable by our carrier.

26.     Interest and willingness to continue personal and professional growth and skill development.

27.     Ability to work with all people regardless of race, gender, religion, age or ethnic background.

 

Other Skills and Abilities Required:

 

The employee fulfilling this job is accountable to the assigned supervisor for completion of assignments and to the high standards of accuracy, attention to detail, and timely completion of work. Expectations include: a pleasant, empathetic, sensitive and professional demeanor, a positive attitude and a willing spirit. The employee must be willing to engage in professional development efforts as available to the position and required by management.

 

Principle Duties and Responsibilities:

 

1.       Responsible for the coordination of all supports in regard to children’s services for developmentally disabled children age 0-17 with a specialty focus on building 0-5 programming.

2.       Assesses the needs of the family and begins the process of person centered planning.

3.       Provides all referrals, linking and coordination of services for families in the Developmental Disabilities network.

4.        Provide individual, group and family treatment to assigned clients.

5.       Maintain a caseload size and weekly productivity expectations as determined by supervisor.

6.       Maintain clinical charts in an accurate and timely manner to include, but not limited to assessments, treatment plans, and all other MDCH chart requirements..

7.       Provide individual, group, and family treatment for children with mental health and co-occurring disorders (developmental disabilities and/or substance abuse).

8.       Monitors and supervises prescribed services and all paperwork involved the process of service delivery.

9.       Maintains responsibility for the clinical record.

10.     Participates in multidisciplinary teams and school advocacy to ensure appropriate service delivery.

11.     Collaborates with the CMH system and the school system to ensure all goals remain in compliance with Medicaid guidelines and are not duplicate in nature.

12.     Through direct contact and correspondence, ensure lifespan has a positive relationship with all referral sources and related agencies.

13.     Follow and comply with clinic policies/procedures.

14.     Must meet the productivity expectations as assigned by program supervisor.

15.     Adhere to all Lifespan policies and procedures as it relates to documentation, training requirements, CMH guidelines and confidentiality.

16.     Cooperates in any investigation related to personnel, licensing, accreditation or other circumstances.

17.     Participate in public relations/marketing activities as needed.

18.     Participate in agency meetings, training and other agency functions as needed or requested.

19.     Attend departmental and agency meetings and trainings as required.

20.     Assist in finding and directing clients in case of an emergency such as a tornado or fire.

21.     Perform other reasonably related tasks as assigned by management or required by the team.

PRE-EMPLOYMENT REQUIREMENTS:   The successful candidate for this position will be required to provide 3 letters of recommendation, satisfactorily complete a drug screen, TB test, national and state background check and DHS clearance (additional items may be required dependent upon position).
