Help Wanted - Registered Nurse/Supports Coordinator – Full Time

Newaygo County Mental Health is seeking a Registered Nurse to provide an array of case management services to adults with severe and persistent mental illness or developmental disabilities.  The basic core case management services consist of assessing, planning, linking, coordinating, monitoring and advocating.  Examples of nursing services would include on-going assessment of physical and behavioral healthcare needs, medication monitoring and education, coordination of care with primary care physician, etc.  In addition, the staff will provide initial/annual assessments, rehabilitative/daily living skill training, support services, assistance in those functional domains impeding community integration, and coordination/monitoring of other authorized services under the plan authorized and are being provided by other internal or external providers.  Services may be provided on or off program site, but most likely in natural community settings where the customer is residing.  The staff in this position must be self-directed and function as a team member.  Registered Nurse with Bachelor’s or Associate’s degree with mental health experience and a valid Michigan driver’s license required.   Salary range: $51,522 - $66,868 based on experience, with excellent benefits.  Send cover letter and resume to: Newaygo County Mental Health, Attn: Human Resources, P.O.
