Case Planner – Infant/Toddler

JOB SUMMARY:

Under the direction of the Shelter/TIL Director, the infant/toddler (0-4 years) case planner is responsible for case management services to infants and toddler in the residential and non-residential programs. The case planner will work with infants and toddlers, and their parents to assure that they are achieving developmental milestones, and those who have been diagnosed with development delays, established medical conditions, and/or specific risk factors. The case planner will work closely with the expecting and/or parent, case planner, and counselor, to ensure that they are part of the coordination of services for the child.

 

ESSENTIAL FUNCTIONS:

 

  • Provide direct intervention services to children 0-4 and their families.
  • Make referrals to educational, medical, and mental health providers.
  • Make all efforts to assure the safety and well=being of every child in the residential and non-residential program.
  • Develop and implement an Individualized Service Plan for the infant/toddler.
  • Coordinate case management services and act as a resource for the child(ren) and families.
  • Assist in transition planning prior to discharge from the residential or non-residential program.
  • Complete weekly individual meetings with the residents in the shelter.
  • Complete weekly home visits to the non-residential participants in their home or community.
  • Promote family involvement in the program.
  • Participate in professional development activities, including staff meetings, conferences, training opportunities, parent meetings, and other training options as identified by the Director of Shelter/TIL.
  • Provide transportation to the parent to required medical, education or mental health appointments in the community. If not available, bus tickets, cab vouchers, or transportation by other community provider i.e. can be arranged.
  • Complete all written progress notes, referrals, and reports.
  • Perform as an effective team member.
  • Perform other duties as assigned.

 

QUALIFICATIONS:  

•           Associate or Bachelor’s degree in Early Childhood Development, Social Work, Education, Nursing, or related Human Service field Preferred. (Note: Due to the mission and requirements of this program, early childhood certifications, education, extensive life and/or work experiences may meet this requirement).

•           Minimally two years of experience working with and knowledge of infants and toddlers ages 0-4 who are at risk of low birth rates and neglect, and infants and toddlers of teen parents.

•           Minimally two years of education and/or experience working with and knowledge of infants and toddlers born premature and/or having cognitive, developmental, nutritional, medical, educational, mental, and emotional problems.

•           Reliability, stability, and consistency in job performance.

•           Ability to effectively, appropriately, and accurately communicate both orally and in writing.

•           Ability to handle crises, maintains confidentiality, and tolerates stress professionally.

•           Willingness to maintain a flexible schedule including evening and weekend hours if necessary, to meet the needs of the residential and non-residential participants.

•           Knowledge of computer word processing and database programs.

•           Valid Michigan driver’s license, excellent driving record and access to personal vehicle.

•           Meet all Agency licensing requirements: TB test, DHS clearance, background check, etc.

•           Ability to give and receive constructive feedback.

•           Ability to work with all members of the community regardless of race, gender, age and cultural or ethnic background.

•           High level of maturity and personal stability; capable of setting limits and organized follow-through.

 

PHYSICAL DEMANDS/WORK ENVIRONMENT:

•           Driving and travel required to attend meetings, training sessions, etc.

•           Moderate lifting of program supplies and donations required on a regular basis.

 

MISC. INFORMATION:

  • Starting Pay Rate: $12.53-$14.97 per hour and is dependent upon education and experience.
  • Part-Time/Temporary (ending June 30, 2017)
  • Work Schedule: 25 hours per week: Three days a week from 9 a.m. to 2 p.m. and two days a week 1-6 p.m. There will be some evenings or weekends if needed to meet the needs of the residential and non-residential participants.
