Community Programs Specialist - part-time

Job Description Summary: The Community Programs Specialist will be a team member in the Community Programs, assisting those who are homeless with accessing housing and supportive services, using a Housing First Model.  

This position will focus on linking program participants with housing options and assisting in securing housing through identification of potential housing leads, working with landlords, reviewing paperwork regarding leases, conducting inspections as applicable and monthly home visits.

 Essential Functions:

 Conduct immediate intake and assessments for homeless individuals during outreach to determine eligibility, short term and long term housing and supportive service needs through Community Programs as well as mainstream resources. Provide basic screening referrals as applicable to alternate programs if the applicant is not eligible for internal programs and provide resources as necessary

  • Maintain contact and document interactions with all program participants on an ongoing basis, utilizing both the case file and HMIS as appropriate. Link to other programs, services and organizations as appropriate in conjunction to services.
  • Provide program participants with hands on assistance to link with resources as applicable. Assistance will include but is not limited to, making the phone call or visit for screening and eligibility with the program participant, providing follow up documentation, and continuing to communicate with assigned service providers to coordinate services.
  • Provide assistance to locate mainstream and community resources in obtaining benefits, birth certificates, social security cards and other important documents necessary for housing and supportive services
  • Assist in efforts to secure housing for program participants by utilizing existing resources within the county. This will include but is not limited to, assisting the program participant in filling out housing applications, locating mainstream resources to assist with security deposits and other housing assistance needs, and providing the limited financial assistance available through Community Programs, as applicable and eligible.
  • Develop an individualized housing case plan for all program participants, while providing consultation to consumers and supportive service providers to develop housing goals and interventions in the person centered planning process.
  • Provide housing related case management on an ongoing basis for program participants, conducting monthly home visits, and working with consumers and landlords to establish positive professional relationships
  • Collect data for the local HMIS to provide statistics regarding homeless persons and outcomes of program services
  • Participate in community-based activities when appropriate such as Community Resource Day, PIT Count, and Human Services Fairs
  • Collect and input data into the local HMIS to provide statistics for all Program Applicants and Participants, as well as documentation of all contacts, assistance and services provided. Information will be entered in a timely manner to insure accuracy of all reports/demographics at any given time
  • Review of leases and other documents related to housing to determine eligibility, including conducting inspections as appropriate.
  • Other duties and projects as assigned

 

 

Qualifications/Requirements:

 

  • Timely completion of required documentation, paperwork, and tasks
  • Compliance with HUD and local governing agency/entity regulations and standards
  • Maintain accurate and up-to-date records
  • Timely submission of Time and Expense reports
  • Maintain equipment in good working order
  • Excellent interpersonal skills and the ability to work collaboratively and effectively, both internally and externally, with individuals with diverse backgrounds
  • Effective communication both in person and telephone interactions and writing skills
  • Organizational and management skills to supervise and guide others
  • Working knowledge of computer programs such as MS Office, internet and databases
  • Experience working with homeless and precariously housed persons and at risk households is preferred
  • Experience working with community agencies and resources
  • High school diploma or equivalent
  • Individuals who are able to communicate and relate a personal experience of recovery are preferred.
  • Candidates must have reliable transportation (as personal vehicles are used for required travel) and adequate insurance coverage (100/300/100 coverage)

 

Physical Demands:

This position does require that a person be able to canvass a variety of location in Outreach which will include both driving to various locations and an ability to travel beyond the vehicle in order to locate potential program participants in parks, under via ducts, in the woods/campgrounds, or other places that those in housing crisis may frequent for makeshift shelter, in all seasons/weather.

 

This position will also require the team member is able to carry some items (under 20 lbs.) with them during outreach and engagement. �Ri��O1U>3'�
