RN/Nurse, Supervisor - Nonprofit Community Health

THE POSITIONs
There are two openings and one is in the the Powell Butte area of SE Portland and the other is in NE Portland. Both work at one of our Secure Residential Treatment facilities.

This site is a secure residential treatment facility serving adults with severe mental illness who have a significant history of institutionalization in the state hospital system. These are transitional facilities. The goal of this program is to work with institutionalized individuals to help them become more independent by learning and using healthy ways of living. Due to the high level of support available, these programs have a higher level of staffing than most residential facilities, including 24-hour nursing care. As a transitional program specializing in assisting individuals in moving to lower levels of support and structure, there is a strong emphasis on community reintegration, utilizing a comprehensive rehabilitation program that includes therapy and skills-building groups, dual-diagnosis treatment, nutrition and exercise, a vocational training program and staff- escorted community outings.

The Nursing Services Supervisor works as a member of an interdisciplinary team to implement and monitor treatment for residents within the treatment facility. This position involves participation and implementation of program goals and objectives to promote a strengths-based, trauma informed client centered approach to assist residents to achieve individual health and wellness goals and ultimately transition to a less restrictive setting. The Nursing Services Supervisor coordinates medical services for the shift and provides psychiatric and medical assessments on an ongoing basis and provides medical interventions. The Nurse Supervisor assumes responsibility for RN treatment decisions and medical procedures in keeping with RN licensure rules and guidelines. Employee supervises medication administration as needed and is responsible for direct supervision of program Certified Nursing Assistant (CNA) or Medication Aid. The Nursing Supervisor participates in communication, medical records, medical supply inventory, communication with pharmacy or other supplier, and works in collaboration with Nursing Services Manager, Nursing team and interdisciplinary team to maintain and improve facility processes.

REQUIREMENTS

Education: 
A BSN or ASN from an accredited college or university.
An active, unencumbered Oregon Registered Nurse License and CPR/First Aid Certification.
Experience: 
A minimum of two years of clinical experience providing inpatient or outpatient services is preferred.
Specialized Knowledge, Skills and Abilities: 
  • Knowledge of medical conditions, medications, and the contexts of care most commonly encountered by people with a severe and persistent mental illness
  • Understanding of the physiological basis for the nursing care delivered
  • Proficiency in using computers and technology (including EMR software)
  • Enter and retrieve data
  • Prepare simple reports
    Other: 
    Ability to commute to various Cascadia sites and community locations as needed.
