Case Manager

(HPI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HPI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages.

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS

The S-BIRT Case Manager shall have, at minimum , a Bachelor's degree from an accredited college or university, specializing in social sciences. The incumbent must obtain and/or maintain current, appropriate licensure/certification issued by the State of Michigan. The incumbent must have at least 1 year of direct professional social service/case management experience, of which 3 months was spent working with adults with SA disorders, within the past 3 years. The incumbent should minimally demonstrate (1) basic knowledge of diagnostics, psychopharmacology, and supportive treatment approaches as applied to a SA adult population; and (2) knowledge of the identification and treatment of co-occurring mental health and substance abuse disorders.

LOCATION: Garden City, MI (Easy access on/off I-96 at Inkster Rd)

SHIFT: Part-Time; Tuesday/Wednesday 5pm-9pm and Sunday 10am-6:30pm

ESSENTIAL JOB RESPONSIBILITIES

Integrated Care:

Substance use screening

Intervention

Referral in a medical setting

Technical

  • Complete Alcohol Use Disorder Identification Test (AUDIT-C) on all patients.
  • Complete AUDIT and/or Drug Abuse Screening Test (DAST-10) on all patients that screened positive on the AUDIT-C.
  • Determine the appropriate level of intervention for patients based on the scores of the AUDIT and DAST-10.
  • Complete the necessary Pre-Admission for all patients who received an AUDIT-C.
  • Complete discharge in Avatar if patient did not screen positive
  • Complete Admission option in Avatar for all patients who did screen positive.
  • Complete admission process including completion of the Project Fit Intake and Project Fit Activity Log.
  • Complete Project Fit Activity Log for all contacts made with the patient.
  • Provide Brief Intervention and Brief Negotiated Interview for all patients who scored in the range appropriate for this level of intervention.
  • Assist the patient with the Prescription for Change goals, if needed.
  • Contact patients 10 days after initial contact and document in Project Fit Activity Log.
  • Provide Brief Treatment to patients that qualify and document all contact in the Project Fit Activity Log.
  • Assist the patient with referrals, contacting supports, payor sources, and community resources and document in the Project Fit Activity log.
  • Assign a Peer Recovery Coach for all patients who qualify for a Referral for Treatment.
  • Complete Discharge once patient has been active for 6 months or per the patient request.
  • If S-BIRT at Hope Clinic, complete the Patient Health Questionnaire during screening.
  • Follow Screening for Depression Policy and Procedure and discuss with primary care Hope physician.

Interpersonal

  • Must be able to conduct all professional business in a manner that is highly ethical and maintains / enhances a professional and positive image of HPI at all times.
  • Must be able to interact effectively and professionally with all internal and external contacts.
  • Assist in building a high-performing S-BIRT team.

Critical Thinking

  • Must be able to differentiate between mental health and substance abuse needs of a patient.
  • Must be able to prioritize responsibilities and tasks in alignment with S-BIRT priorities and goals.
  • Must be able to provide high quality services in a cost effective and time efficient manner.

Physical Requirements

  • Must have adequate visual acuity to utilize a PC
  • Must be able to drive periodically to HPI clinics and local community resources.
  • Must have adequate visual acuity to interact with the Electronic Health Record System and accurately input data.
  • Regular and reliable attendance and punctuality is required.
