The Care Manager provides care management and care coordination for adults and children with complex health and serious mental illness/serious emotional disturbance (SMI/SED). Provides services in the community and in consumers' homes, under minimal supervision. Assesses, implements, coordinates, monitors, and evaluates all options and services with the goal of optimizing the consumer's health status. Integrates evidence-based clinical guidelines, preventive guidelines, and protocols in the development of individualized care plans that are consumer focused, promoting quality and efficiency in the delivery of physical and behavioral health care. Provides targeted interventions to avoid hospitalization and emergency room visits.



Graduation from a school, approved by the State Board of Nursing having one of two types of nursing education programs, diploma or associate degree and two years of experience OR possession of a Bachelor's of Science Degree in Nursing and one year of experience OR possession of a Master's Degree in Nursing. Experience working with individuals with mental illness or developmental disabilities preferred. Michigan nursing license and valid driver's license required. CPR certification preferred.
